Our insurance coordinators deal with many different insurance companies. Some companies offer hundreds of different dental and medical plans. Some companies combine dental and medical coverage. Policies and guidelines may change weekly. Eligibility may change daily  based on your hours worked and your employment status. Even though we have real-time access to many insurance companies online, it is not possible to positively confirm your eligibility nor accurately estimate our patients’ insurance co-payments. Many insurance companies will not give out fee estimates before the treatment is completed. Dealing with these companies can be difficult and time consuming. To maximize your reimbursement, it is important that all information about you and your insurance be current and accurate. 

Co-Payments – All co-payments are payable at time of service or 1 week prior to surgery.  Your co-payment is an estimate based on information provided by your insurance company and is not a guarantee of your of coverage or benefits.  Your co-payment is a deposit towards the total and your full financial responsibility will be determined after your claims have been processed by the insurance company.

HMO Patients – For problems that may be covered by medical insurance (e.g. biopsies, tumors, TMJ problems, infections, jaw deformities), patients must have an authorization from their primary care physicians. A referral from a dentist is not adequate. Obtaining a medical authorization is the patient’s responsibility. We cannot obtain the authorization for you, and the authorization cannot be obtained retroactively. If you do not have an authorization, we will be happy to see you on a cash basis, but your medical insurance company will not pay for your treatment.

Private and Group Insurance – As a courtesy, we will file your insurance claims for you. In some cases, we may ask that you pay for the treatment rendered and we file your insurance claim for your reimbursement.
